Community and Outreach Services for Adults

Designed to enable adults to live as independently as possible in the community, Steininger offers a wide range of treatment, rehabilitation and assistance for individuals with serious and persistent mental illness who need to reintegrate into the community.

Steininger’s Partial Care program, located in Berlin, is a day treatment program for individuals with a serious and persistent mental illness who need daily intensive treatment and skills building. Clients attend two to five days a week for five hours of programming designed to teach activities of daily living and work readiness skills. We provide an intensive level of outpatient behavioral health treatment (IOP) for adults who are experiencing symptoms of a mental illness. These symptoms interfere with everyday life activities and may place them at risk of needing crisis intervention, psychiatric hospitalization, or residential services. Our goal is to give people the treatment they need to restore their quality of life. Our IOP is designed to meet the individual needs of its participants. Services and can benefit those adults who have never been in treatment previously, as well as, those with past treatment. For individuals recently discharged from hospitals or other residential settings, our IOP helps continue their mental health treatment in the community.

Chemical Abusing Mentally Ill Case Management                              (856)427-0400

A range of specialized case management services is provided to individuals whose substance abuse behaviors have led to serious mental health problems. These services include assessment, individual counseling, outreach, referral/service coordination, after care planning, and case management follow-up and advocacy. Steininger Case Managers also help individuals build the skills required to manage daily living and reduce their reliance on substances of abuse.

Steininger Case Managers assist those individuals with serious and persistent mental illness who have difficulty managing their money by teaching them how to budget funds effectively and develop greater self-reliance. As part of this program, Steininger can be designated as the representative payee for Social Security Insurance (SSI) and Social Security Disability (SSD) payments.

Correctional Case Management                                                          (856)427-0400

Steininger provides specialized case management services that help inmates suffering from mental illness transition smoothly back into the community from the Camden County Correctional Facility. This program is designed to prevent re-admission to the correctional system or psychiatric hospital. Through the intervention of Steininger professionals who devote their efforts to those inmates needing linkages to community resources such as housing, medication, outpatient treatment and case management.

Steininger’s Integrated Case Management services assist individuals in living successfully in the community by providing: assessment; outreach; supportive counseling; linkage with mental health, medical, financial, housing, and educational/vocational services; service coordination; and advocacy. These services are also available to people with the coexisting disorders of mental illness and substance abuse.

Programs of Assertive Community Treatment                         (856)428-7632

This program offers comprehensive, integrated treatment, rehabilitation and support services to adults who are most challenged by the need to cope with serious and persistent mental illness. These services are provided by a multi-disciplinary team of professionals that assist these individuals in maintaining active participation in the community and increasing their quality of life. Services to an individual may vary in time and intensity, and the treatment has no predetermined ending point.

Projects for Assistance in Transition from Homelessness          (856)427-0400

PATH case managers reach out to rescue missions, food centers, and other places where people with a mental illness who are homeless congregate. Services include linking people to shelters or permanent housing as well as social services and mental health supports.
